Thank you for allowing us to care for your pet(s). We truly strive to provide the best care and facilities in the area, and that is why we require all animals to be current on vaccines and free of intestinal and external parasites. Your pet(s) will experience an environment different from your home, unfamiliar people, and other unfamiliar and sometimes noisy pets. As a result, it is possible that your pet(s) may experience stress induced vomiting, diarrhea, hoarseness or a sore throat (especially if your pet(s) is prone to excessive barking). We promise to keep your pet(s) as comfortable as possible and treat him/her as needed for these symptoms. Please rest assured that we treat every pet entrusted to our care the same way we treat our own pets. Our own pets board in the clinic when we are out of town just as your pets do

I understand you CANNOT guarantee the health of my pet. I understand and will not hold the clinic responsible for conditions that are unavoidable in boarding kennels, such as but not limited to weight loss, hair loss, upper respiratory infections, bronchitis and diarrhea. I understand that in the event of my pet’s illness, the staff will immediately try and contact me or my emergency agent.

In the event that contact is unable to be made, I authorize the doctors of Battlefield Animal Clinic and their medical staff to sedate my pet and/or perform such emergency procedures as deemed necessary for the health and welfare of my pet until myself or my agent can be reached. I agree to pay, in full, all charges for necessary services rendered for and to my pet. The clinic is to use all reasonable precaution against injury, escape, or the death of my pet.

The clinic and its staff will not be held liable for any problems that develop provided reasonable care and precautions are followed. I will call if my “pick-up” date changes so that you can plan accordingly. If I neglect to pick up my pet within 5 days of the date scheduled for discharge, and do not notify you within that time period, you may assume that the pet has been abandoned and are hereby authorized to dispose of the pet as you deem best and/or necessary.
